How to allocate support to Heritas LT?

Log in to the Electronic Declaration System.

Select form to fill in FR0512 version 4.

Fill in the personal data and the following fields in capital letters on the form:
- 5 The tax period is 2022
- 6S I allocate part of the fee to the beneficiaries -
- E1 Beneficiary type - 2 (beneficiary)
- E2 Beneficiary identification number (code) - 305441813
- E4 The amount of the tax part (in percent) - can be up to 1,2
- E5 I allocate part of the tax until the tax period - by 2025 at the latest. (to be completed only for support longer than 1 year)
